The documents requested for the drafting of a certificate :
* A copy of the Lebanese passport.
* A copy of a recent Individual and Family Extracts of Civil Status.
* A copy of the residence permit. If available, a copy of the identity card or swiss passport.
* Full names of the father and the mother in French.
* Self-addressed stamped envelope.
